The Activity Submission form is not an official part of the Blue Flag application. This is a form which will help you to review your activities as they happen and create a log of activity information you can use when filling out your application. By filling out the form below you are consenting to staff using your activity information for national promotional use. With this in mind, if you need media release forms please reach out to staff for a copy.
The form asks for the information of the submitter and the activity contact. If your organization does not run the event you are submitting, please include the contact information for one of the responsible organization’s representatives who ran the activity.
